 RECORD REVIEW INSTRUCTION PACKET

INSTRUCTIONS FOR SUBJECT OF RECORD TO OBTAIN A COPY OF THE INFORMATION IN HIS/HER STATE CRIMINAL RECORD

  • The Criminal History Records Section of the Arizona Department of Public Safety serves as the Central State Repository for criminal records in the State of Arizona (see Arizona Revised Statute 41‑1750).
  • The subject of a criminal record may review the information contained in the record for the SOLE PURPOSE OF REVIEWING FOR THE ACCURACY AND COMPLETENESS OF THEIR RECORD.
  • Criminal history records will contain no personal identifying information (such as name, address, etc.).
  • If the request is for the purpose of employment, employers may contact the Applicant Team at (602) 223-2223, 8 a.m. to 5 p.m., Monday through Friday.
  • If you believe the information in your criminal history record is either inaccurate or incomplete you may request a Record Review Packet.
    Contact the Criminal History Records Section at (602) 223‑2229, 6 a.m. to 2 p.m., 7 days a week.
  • The Record Review Packet contains the following items:
    • Instructions
    • Blank fingerprint card
    • Contact information sheet
    • Pre-addressed mailing envelope for completed fingerprint card
  • A full set of legible rolled fingerprints will be required.
  • This procedure ensures positive identification that the individual requesting the record review will receive the correct criminal history record.
  • A completed fingerprint card cannot be processed if it is bent, folded or if the fingerprint impressions are not clear.
  • Submit the completed fingerprint card to:

    Arizona Department of Public Safety
    Criminal History Records Section
    Attn: Dayshift Supervisor
    P.O. Box 18450
    Phoenix, AZ 85005-8450

  • If an individual is being represented by an attorney, the attorney must submit a notarized letter of authorization from the individual authorizing the attorney to review the individual’s criminal history record.
  • The Criminal History Records Section will mail an acknowledgement to the Requester confirming receipt of the fingerprint card.
  • Upon receipt of Record Review Request, a copy of any existing criminal history record will be mailed in approximately 15 days.
